PHOTO RELEASE: I hereby grant to Ronald McDonald House Charities (RMHC), it’s local chapters and programs, advertising, and promotional agencies, and their agent (collectively, “RMHC”), the irrevocable, unrestricted right to use, publish, display and distribute materials bearing my name, voice, likeness, or any other identifiable representation of myself. These materials may appear in any form, style, color or medium whatsoever now or hereafter known (including without limitation, photographers, videotapes films, sound recordings, software, drawings, prints, broadcast, internet, and electronic media).
I agree that all materials containing an identifiable representation of me (including, without limitation, all negatives, plates and masters of any photographs, files, prints, or tapes) shall be and remain the sole and exclusive property of RMHC, and I hereby assign any proprietary right I may have acquired in or to such material to RMHC. I hereby release and forever discharge RMHC from any all liability and damages relating to the use of my name, voice, likeness, or any other identifiable representation of me.
I hereby waive any right I may have to inspect or approve the finished materials or any part or element thereof that incorporates my name, voice, likeness or any other identifiable representation of myself.
I have agreed to the above in consideration of the opportunity given to me by RMHC to appear in these materials. I acknowledge that I have fully read and understand this document and that I have had any questions regarding its effect or the meaning of its terms answered to my satisfaction. I certify that I am at least 18 years of age, unless this document is also signed by my parent or legal guardian.

This confidentiality policy explicitly states what information employees, members of the Board of Directors, and volunteers are prohibited from disclosing during and after their employment or volunteer experience
CONFIDENTIAL INFORMATION
The employees, members of the board, and volunteers of the Ronald McDonald House of the Greater Hudson Valley (RMH of the GHV) manage and have access to confidential information that must stay within our organization. Confidential information includes, but is not limited to, our guest families, donors, supporters, employees, marketing processes, as well as our financial information, which includes current and future business plans, our computer and software systems and processes, personnel information, and associated documents. Employees, board members and volunteers are not permitted to share this confidential information with anyone outside the organization, or to remove or make copies of any of the RMH of the GHV records, reports, or documents in any form, without prior management approval. Disclosure of confidential information may lead to disciplinary action, which may include termination of employment or volunteer opportunity, as well as other possible legal action. Additionally, employees, board members and volunteers of RMH of the GHV are prohibited during and/or after employment and volunteer experience from using RMH of the GHV’s confidential information in any form for their own purposes or for those of other persons or entities. Finally, all confidential information relative to RMH of the GHV, regardless of its form, must be returned to the organization at the time of termination of employment or volunteer opportunity with the organization.
I am aware that, during the course of my employment or volunteer opportunity, confidential information will be made available to me. Further, I understand that this information is proprietary and critical to the success of RMH of the GHV and may not be distributed or used outside of RMH of the GHV premises or with non-RMH of the GHV individuals. In the event of my termination of employment or volunteer opportunity, whether voluntary or involuntary, I hereby agree that I will not utilize or exploit this information for my own personal gain, or share it with any other individual, nonprofit agency, or company.
ASSUMPTION OF RISK
I hereby release and forever discharge and hold harmless Ronald McDonald House Charities (RMHC), it’s local Chapters and programs, advertising and promotional agencies, and their agent (collectively, “RMHC”) from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my activities with RMHC. I understand that this release discharges RMHC from any liability or claim that I may have against RMHC with respect to bodily injury, personal injury, illness, death, or property damage that may result from my activities with RMHC, whether caused by the negligence of RMHC or its officers, directors, employees, or agents or otherwise. I also understand that RMHC does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance in the event of injury or illness. I hereby expressly and specifically assume the risk of injury or harm in the activities and releases RMHC from all liability for injury, illness, death, or property damage resulting from the activities.
